Autopilot was accidentally left on
Ship crashed into the thing it was meant to survey
Evacuation occurred without ships extensive fire suppression systems being activated properly
Ship then sank
All in all, it was a massive cock up for her

The Captain involved is English, Yeeb. Trained by the Royal Navy and migrated to NZ with her British wife in 2012.

That will undoubtedly be the case , very likely statistically a straight white man responsible for the fail too. The buck stops with the captain though , and it will be only her castigated and mocked for this incident.
It is quite a sad world that someone saw appropriate to mention her sexuality in the article title, am astounded the editor there thought it was ok or relevant. Joking aside this will annoy the bobble heads from both sides, the dinosaurs who beleive woman and gay women especially should have no place in a navy position of command , and the bobble heads on the other side of the spectrum who think it’s unreasonable to makes every joke under the sun for the multiple failures of command or get upset when it’s mentioned.
I also actually feel sorry for other women (gay or not) in the NZ armed forces , as this event does raise reasonable question marks of competence, promotion reasons, box ticking , and may be an inadvertent barrier to their own careers.
Maybe worth waiting for the results of the investigation before apportioning blame. Without knowing anything about the kiwi navy it doesn't seem that difficult to believe she might have been the best candidate available to them, despite being lesbian handicapped
