Also unlucky. Whatever happens here, England had ample opportunity between minutes 40-60 to completely kill the game. That they didn't was fucking awful.

Itoje and co. got away with that a bit, but Fiji have allowed themselves to have the man sacked at every lineout. Dumb not to adapt.
