Mars Wrigley factory fined after two workers fall into chocolate vat

US workplace safety regulators have fined a Pennsylvania factory after two workers fell into a vat of chocolate and had to be rescued.

The Mars Wrigley factory in the city of Elizabethtown was fined more than $14,500 (£12,000) by the Occupational Safety and Health Administration.

The workers were contractors that did not work full time for the factory.

The incident happened in June 2022. A hole had to be cut into the bottom of the partly-full tank to get them out.

More than two dozen rescuers responded, and one worker was transported to hospital by helicopter, according to local reports.

The regulator's report labelled the incident "serious". It says the workers were hired to clean tanks, and were not provided with proper safety training.

In a previous life I was in the factory ERT, & because I'm a short arse; I was part of the, "Confined Space Team".

This is all standard stuff, & OSHA & other industry bodies have really, really clear definitions on this, & how working in these areas requires special training, because they are so often far more dangerous, because it's fundamentally more difficult to get in & out of them.

$15k is a fucking derisory sum of money to fine a company that large, they spend that much on a single mid level executive trip to an off shore factory.

For an incident like that, where the company didn't give 0.00001% of a fuck about the life of a contractor, they should star the fine at, say 10% of the previous years Corporate profits, & if there is a fatality, then it immediately becomes 100% of the previous years profits, etc, etc.

Start to wipe out their profits & they'll start to get serious about human life.

I believe there are legal upper limits on what the fine can be..

EDIT: I just checked with a friend who is highish up in OSHA and apparently, that fine is the maximum permitted for that violation classification. Compensation for the families will be through private litigation.

So we have a server that performed its own automated supplier-invoked update to one of their services.
As a result of this update, it broke their admin console used to admin the software.

I logged a call about this with their support on 3 Feb.
The first support idiot annoyed me no end, was unable to resolve the issue and it was escalated on 7 Feb.

Since then we've had 2 lengthy remote control teams sessions to attempt to resolve this without any success, with the engineer concluding that there was something wrong with the server's .NET installation it was therefore out of their remit to resolve and it was not the application...

So I built a new clean server, installed the .NET required for their application and performed all the applicable windows updates. I managed to prove that the .NET issue he thought was the cause was exactly the same on this clean build. (2019 server & dotnet-core-uninstall powershell tool, doesn't list any .Net installs which seems to be an undocumented Microsoft 'feature' of the script)

I proceed to then install their latest admin app installer on the server and launched the broken admin tool in question on it. It launched without any of the nasty errors we were getting on the production server. It asked for SQL connection details and I was able to connect to the prod server SQL database without issue. OK so maybe it does have something to do with .NET afterall... or not.

What I did notice is that admin app didn't save a .config for the connection details in the launch folder (the associated service on prop server does). So I rooted around and found a new refresh config.xml in a different folder path on the server (after first searching the registry to if it was saved there). Turn out some config are saved in \Program Files\ others randomly in hidden \Program data\ because programmers are idiots.

So I copied this good working xml file from temp server to production server and HEY PRESTO it worked.

2weeks and at least 10+ hours fucking around with .NET install/re-install/remove/upgrade/update/repair etc etc - reboot after reboot and even a full server recovery from backup after it went properly tits up.

but it was their simple fucking xml file... FML

I spent an hour yesterday listening to colleagues bitching about MS deep sixing support for a plethora of old Client, & Server versions. MS don't give a shit, & to an extent I understand, but there is a colossal installed base of XP -> Windows 7 clients out there that no-one wants to replace.

A major US food sanitation company illegally employed at least 102 children between the ages of 13 and 17 in jobs that had them using caustic chemicals to clean razor-sharp saws.

Packers Sanitation Services paid $1.5 million in civil penalties for employing minors in hazardous occupations and having them working overnight shifts at 13 meat processing facilities in eight states, a US Department of Labor investigation found.

The Kieler, Wisconsin, based company employed the children to clean meat processing equipment including back saws, brisket saws and head splitters. At least three children were injured, investigators said.

The company was fined the maximum civil penalty allowed by federal law — $15,138 for each minor-aged employee “who was employed in violation of the law.”

We had trains that were too long for the platforms meaning that they had to close the station to rebuild.....

And the new Metro tunnels being bored have been dug 40mm too narrow to ever change to default rolling stock - part of the Govt plans to remove drivers and guards from service

A drunk 14-year-old boy sparked a mass brawl after he smashed a kebab and chips out of man’s hands.

Police received multiple calls over disorder on George St, near Dunedin’s Octagon, just after bars closed on Sunday about 3.15am, Senior Sergeant Anthony Bond said.

Officers were in the area and witnessed a brawl between 10-15 people outside a kebab shop, including one male being kicked in the head and body by three other males.

Pepper-spray was used to prevent further offending and disperse the crowd, Bond said.

An intoxicated 14-year-old was identified as the instigator of the incident, and was taken home and referred to Youth Aid.

The incident involved the teen knocking the kebab and chips out of the hands of a 19-year-old man.

The 14-year-old was also one of the three who kicked a person on the ground, he said.

A few minutes later police attended a crash, after a 22-year-old female decided to drive home after drinking in the Octagon.

She drove-up Stuart St at speed and collided with a street lamp near York Pl, causing the car to plunge down a bank and into the Jehovah Witness’ church car park.

The woman was taken to Dunedin Hospital, and refused a blood sample.

She was forbidden to drive for 28 days, and was summonsed to appear in court for refusing to give blood.

More charges were likely, he said.

That's rare enough. I'm definitely more conservative on some issues and would have been actively left wing when younger. My job years ago brought me into contact with people that opened my eyes to a lack of individual and community responsibility. So it would be mainly conservative on issues of personal responsibility. The problem for me is how binary it has become as in if you are dubious about the type of supports for long term unemployed you must be against gay marriage. Also the Ham fisted attempts to import shit from other countries that isn't relevant. I saw a debate online about a particularly Irish concern the other day and one lad says 'of course you SJW Biden supporters would support this' and the other lads accuses him of Trumpism. What the actual fuck has that got to do with anything in Ireland

Apparently she got into an argument with a fellow passenger who objected to this “professional beggar”

It went on until the passenger said, “He’s only going to spend it on booze”

The young woman replied, “so was I”
